- Completed Intake paperwork and/or Questionnaire. Contact office for instructions to complete online through the Patient Portal and/or download Intake Paperwork, Questionnaire, and Privacy Disclosures.
- Insurance Card
- Driver’s License/Picture ID
- List of current medications and allergies
- List of prior medications
- List of all prior surgeries or procedures
- Pharmacy Information
- MRI, X-Ray, or CT Reports if available
