based upon individual patient needs. Patients are seen at the Pain Diagnostics and Treatment Center on a physician referral basis.
If you are a possible patient who would like to be evaluated by our office, please discuss this

Physicians may refer a patient to our office by telephone or by fax, whichever is easiest
for your office. We have developed a fax referral form to assist your office staff, using information
